Клінічні особливості профілактики ускладнень та перебігу міопії під впливом комбінованого методу лікування: фотобіомодуляції та тривалої (6 місяців) нутрієнтної терапії

Резюме

Актуальність. Проблема короткозорості залишається актуальною через збільшення частоти випадків, схильність до серйозних ускладнень. Мета: визначити клінічні особливості перебігу міопії середнього ступеня та методи профілактики ускладнень шляхом застосування комбінованого методу лікування — фотобіомодуляції (ФБМ) та тривалої (6 місяців) нутрієнтної терапії. Матеріали та методи. Обстежено 52 особи (104 ока) з міопією середнього ступеня. Хворі були розподілені в 2 групи: 27 (54 ока) — основна та 25 (50 очей) — контрольна. Усім 52 пацієнтам був проведений курс фотобіомодуляції на діодному лазері СМ-4.3 (λ = 650 нм, I = 0,4 мВт/см2, експозиція 300 с). 27 пацієнтам основної групи був рекомендований вітамінно-антиоксидантний комплекс формули AREDS, посилений вітаміном D3, омега-3 ПНЖК та ресвератролом. Усім пацієнтам було проведено стандартні загальноклінічні й офтальмологічні обстеження. Результати. Значних відмінностей динаміки показників після курсу ФБМ між групами не було. Однак спостереження через 6 місяців визначили, що показник гостроти зору (ГЗ) був на 23 % вищий у групі короткозорих, які отримували вітамінно-антиоксидантний комплекс протягом 6 місяців, ніж у контрольній групі; також за результатами частотного аналізу в групі, яка отримувала нутрицевтик, наприкінці дослідження в 3,1 раза частіше визначалась ГЗ, що була вищою за 0,3. Усі інші показники через 6 місяців спостереження в групі без нутрієнтів поверталися на рівень до лікування. У групі на фоні тривалого 6-місячного прийому вітамінно-антиоксидантного комплексу формули AREDS з вітаміном D3, омега-3 ПНЖК і ресвератролом відмічена стабілізація резервів акомодації (РА) на 84 % вище, ніж до лікування. Стабілізація колбочкової світової чутливості, варіабельність показника якої була зменшеною у 2 рази на початку дослідження, після лікування на 7-й хвилині досягла нормальних значень. Також у групі з нутрієнтами поліпшилися показники реофтальмографії завдяки стабілізації об’ємного кровонаповнення ока та значному зниженню спазму внутрішньоочних судин (α/t1) на 17,4 % (р < 0,05) через 6 місяців спостереження. Аналіз даних пупілографії наприкінці спостереження виявив зменшення максимальної та мінімальної площі зіниць під час акомодаційної конвергенції в середньому на 13 %, що свідчить про нормалізацію балансу функціонування симпатичного та парасимпатичного відділів ВНС, що іннервують циліарний м’яз, на фоні тривалого прийому вітамінно-антиоксидантного комплексу формули AREDS, посиленого вітаміном D3, омега-3 ПНЖК і ресвератролом. Висновки. Курс лікування, що містить застосування комбінованого методу лікування: фотобіомодуляції та тривалого 6-місячного прийому вітамінно-антиоксидантного комплексу формули AREDS з вітаміном D3, омега-3 ПНЖК і ресвератролом, дозволяє значно поліпшити гостроту зору, підвищити резерви акомодації, знизити силу оптичної корекції, стабілізувати внутрішньоочний кровообіг та знизити спазм внутрішньоочних судин крупного калібру шляхом нормалізації балансу функціонування симпатичного та парасимпатичного відділів ВНС та регуляції трофічних механізмів з поліпшенням фотопічної світлової чутливості. Завдяки цьому даний курс терапії сприяє профілактиці ускладнень перебігу міопії середнього ступеня.

Background. The problem of myopia remains relevant due to an increase in its frequency and the tendency to serious complications. Purpose: to determine the clinical features of the course of moderate myopia and methods to prevent complications by using a combined method of treatment — photobiomodulation (PBM) and long-term (6 months) nutrient therapy. Materials and methods. Fifty-two people (104 eyes) with moderate myopia were examined. They were divided into 2 groups: 27 (54 eyes) — main one and 25 (50 eyes) — controls. All 52 patients underwent a course of PBM using a SM-4.3 diode laser (λ = 650 nm, I = 0.4 mW/cm2, exposure of 300 sec). Twenty-seven people of the main group were recommended the vitamin-antioxidant complex of the AREDS formula, enhanced with vitamin D3, omega-3 polyunsaturated fatty acids (PUFA) and resveratrol. All patients underwent standard general clinical and ophthalmological examinations. Results. There were no significant differences in the dynamics of indicators after the PBM course bet­ween the groups. However, a follow-up after 6 months determined that visual acuity was 23 % higher in the myopic group receiving the vitamin-antioxidant complex for 6 months than in controls; also, according to the results of the frequency analysis, in the group taking a nutraceutical, the visual acuity higher than 0.3 was determined 3.1 times more often by the end of the study. The data of all other indicators after 6 months of observation in the group without nut­rients returned to the level before treatment. On the background of a long-term 6-month intake of the vitamin-antioxidant complex of the AREDS formula with vitamin D3, omega-3 PUFA and resveratrol, a stabilization of the accommodation reserve was 84 % higher than before treatment. Stabilization of cone spectral sensitivity whose indicator of variability reduced by 2 times at the beginning of the study, after the treatment at the 7th minute has reached normal values. Also, in the group with nutrients, the indicators of rheophthalmography were improved due to the stabilization of the volumetric blood filling of the eye and a significant decrease in the spasm of intraocular vessels (α/t1) by 17.4 % (p < 0.05) after 6 months of observation. Analysis of pupillography data at the end of observation revealed a decrease in the maximum and minimum pupil area during accommodative convergence by an average of 13 %, which shows the normalization of the balance of the functioning of the sympathetic and parasympathetic parts of the autonomic nervous system that innervate the ciliary muscle against the background of a long-term administration of the vitamin-antioxidant complex of the AREDS formula, enhanced with vitamin D3, omega-3 PUFA and resveratrol. Conclusions. The course of treatment, which includes the use of a combined method of treatment: photobiomodulation and a long-term 6-month intake of the vitamin-antioxidant complex of the AREDS formula with vitamin D3, omega-3 PUFA and resveratrol, allows you to significantly improve visual acuity, increase accommodation reserves, reduce the power of optical correction, stabilize intraocular blood circulation and reduce spasm of large intraocular vessels due to normalization of the balance of functioning of the sympathetic and parasympathetic parts of the autonomic nervous system and regulation of trophic mechanisms with improvement of photopic light sensitivity. Thanks to this, such course of therapy contributes to the prevention of complications of moderate myopia.
